We are proud to celebrate our second year in earning formal recognition through The Joint Commission as a Primary Care Medical Home. In celebration of this we would like to provide some insight as to what this entails. The definition of PCMH (Primary Care Medical Home) is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive the necessary care when and where they need it, in a manner they can understand. As a PCMH we at Kernodle Clinic want to partner with you to ensure all your physical and mental healthcare needs are met. Our clinic is available to coordinate and manage any care that is associated with Duke Health, which includes specialty care, community services, hospitals, and home healthcare services. You as a person and your wellbeing are the center of everything we do.
